Referral Form

Welcome.
Relax.
Smile.

What to expect on your first visit.

While some of our patients are referred to us through our sister clinic, Georgian Mall Family Dental, other patients come to us by referral from other general dentists, dental specialists and health care professionals outside the dental sphere. We are also delighted to accommodate patients who would like to visit us without receiving a referral. New patients always welcome!

Patients With Referrals

If you have been referred to Georgian Mall Dental Group, please contact us during office hours to schedule an appointment personally with a care team member. Our office can then contact your dentist directly to get the information (and radiographs) necessary for your initial consultation. As standard practice, we are committed to maintaining a good and open line of communication with your dentist to ensure you receive the full 360-degree dental care you deserve.

Your first visit with:

Orthodontist

Your First Visit with an Orthodontist

Whether you’re getting orthodontic work for yourself or your child, we’re here to make sure you feel totally comfortable and at ease with your future treatment.

At your first appointment, we’ll start by determining if orthodontic treatment is even needed and, if so, how and when to start. This involves conducting a thorough examination of the patient’s mouth, and taking photographs and x-rays to assess treatment requirements. We’ll then offer our recommend treatment and, if there is more than one treatment option available, we’ll carefully explain the options including the pros and cons of each.

If we recommend an appliance as part of the treatment, we’ll explain how the appliance works; we’ll also tell you how long the full treatment program will last and your anticipated frequency of appointments. Finally, we’ll provide you with an estimate of the treatment costs, review your insurance coverage and discuss all your other financing and payment options. Please note, all patients under 18 must be accompanied by a parent or guardian.

Oral Surgeon

Your First Visit with an Oral Surgeon

As a patient-first clinic, your absolute comfort is our first priority. At your initial visit, your appointment with your Oral Surgeon begins with a full consultation. During this session, we’ll explain your diagnosis, why surgery is necessary and review your treatment options.

Sometimes, we’re able to perform your surgery right away that very day. However, if you require more complex work, we’ll invite you back for a second appointment. If you are receiving general anesthesia for your surgery, you must have an adult with you to accompany you home.

Please make sure you bring the following to your first appointment:
• Your referral slip
• X-rays from your referring dentist (if applicable)
• A complete list of any medications you are currently taking (including any herbal remedies)
• Dental insurance information (if applicable)

Please note, all patients under 18 must be accompanied by a parent or guardian.

Endodontist

Your First Visit with an Endodontist

In most cases, endodontic treatment is a relatively simple and straightforward procedure, so we’re often able to perform your root canal in the same session as your initial consultation. Your first visit with your Endodontist will begin by reviewing your medical history and any medications you’re currently taking. Your Endodontist will also carefully explain the procedure and why it is necessary, and address any questions or concerns you might have.

Once you feel comfortable with the procedure, we will begin treatment. General anesthesia for endodontic treatment endodontic treatment is usually not required, so you should be able to leave this appointment soon afterward and get back to your normal daily routine.

Periodontist

Your First Visit with a Periodontist

Our primary concern during your first visit with your Periodontist is to ensure you understand what your treatment entails and answer all your questions. Before beginning our work, we will complete an in-depth exam – to look for loose teeth, bite issues, TMJ, measure for bone loss, and visually screen for oral cancer.

We’ll also look for other signs of periodontal disease, in and above the reason for your referral. As a certified Periodontal Specialist, your doctor will complete your exam as gently as possible, with little or no discomfort.

Prosthodontist

Your First Visit with a Prosthodontist

Every patient and every prosthodontic treatment is different. That’s why your GMDG Prosthodontist will sit down with you to thoroughly review your medical history, and listen carefully to your concerns and expectations. If your prosthodontic needs are relatively straightforward and all your questions are answered to your satisfaction, we’ll use this first visit to conduct a general dental examination (including radiographs if needed) and review your treatment options along with estimated fees, number of appointments needed and any post-operation care. However, if your problem is more complex, we may require additional appointments to complete your full consultation and treatment.

Please make sure you bring the following to your first appointment:
• Your referral slip
• X-rays from your referring dentist (if applicable)
• A complete list of any medications you are currently taking (including any herbal remedies)
• Dental insurance information (if applicable)

Pediatric Dentist

Your First Visit with an Pediatric Dentist

Nothing brightens up our clinic like happy children with shiny, radiant smiles. That’s why we aspire to make your child’s first visit to GMDG the happiest part of their day, and something to look forward to for many years to come! Of course, your comfort is just as important to us as your child’s. We’ll kick off your first visit with a short tour of our office, to explain our on-site amenities and some of the exciting new technologies we’ve incorporated into our clinic. Then, we’ll invite you and your child into one of our private exam rooms, where they can sit back in a magical chair and enjoy their favourite TV show or movie. (Psst: meanwhile, we’ll gently examine their current dental health and provide any necessary care such as cleaning, flossing and future-proofing their teeth for a lifetime of smiles.) But our work today is only the tip of the iceberg – we’ll also review dental care techniques you should be practicing at home every day, twice a day, to ensure your child’s teeth are just as clean and healthy for your next visit!
